CONSCIOUSNESS


Mainly two types of consciousness.
1. Clear consciousness (Normal state)
2. Affected consciousness

We have 4 stages of affected/Altered consciousness.
1. Clouding of conscious
2. Stupor
3. Sopor
4. Coma
Let discuss briefly in this blog!

Clouding of conscious:
        In this stage, the patient can't answer immediately. It is undifferentiated to patient condition.

Stupor:
        In this stage, the patient will be in constant sleep. He/She may answer yes or no to the questions after the aloud or painful stimulus. May poorly orient to environment, place, time and sometimes persons.

Sopor:
        Subject in a severely impaired consciousness. He/She can't response to others. Injection can deduce from this condition but for a short interval of time.

Coma:
It is unconscious statetotal absence of response to external stimuliabsence of reflexes and disorder of the vital functions.

Glasgow coma scale and score:
From the details in above table, we can calculate the mental conscious scoring of a subject out of 15.

For example,
Subject name: XXXX
Date : DD/MM/YYYY

Glasgow coma sore:
Eye opening             - 3 ( means subject can open eyes by once speech)
Verbal response       - 4 (Confused conversion)
Best motor response - 5 (Localized pain)

Total score : 12/15 (Hence, the subject is not in coma stage)

Note!
Severity < 9    Moderate 9 - 12    Minor ≥ 13

Classification of Coma:
1. Metabolic coma
2. Toxic coma
3. Cerebral coma

Metabolic Coma:
It includes
1. Hypoglycemic coma,
2. Hyperglycemic coma  (diabetic ketoacidosis),
3. Coma in hypothyroidism,
4. Thyrotoxin coma (acute phase of thyrotoxin crisis),
5. Hepatic coma,
6. Uremic coma,
7. Hypoxemic coma,
8. Coma in adrenal insufficiency.
Toxic coma:
It includes
1. Alcoholic coma,
2. Coma in morphine intoxication,
3. Coma in over dosage of another drugs (nootropic),
4. Coma in intoxications by industrial poisons.
Cerebral coma:
It includes
1. Apoplexy (apoplectic coma),
2. Brain carcinoma,
3. Cerebral trauma,
4. Cerebral infections,
5.Vasculitis (in systemic diseases),
6. Epilepsy (epileptic coma).
